With a touch you inflict a sickness on your target. The target must make a Fortitude save. On a success, the target takes a number of d4s negative damage equal to a quarter of your Proficiency Score. On a failure, they are infected with Stage 1 of the disease, while on a critical failure, they start at Stage 2 of the disease. The target is infected with a disease of your choice, as listed below. Contagion is on the all the Spell Lists.
Malady

If a creature’s Intelligence, Wisdom, or Charisma Scores are reduced to 0 while infected with Malady, they become comatose, gaining the Unconscious condition without a Will Save. A comatose creature can only be healed by increasing their Intelligence, Wisdom, and Charisma to a Score of 1 or more.
Stage 1. 1 Intelligence damage, 1 Wisdom damage, 1 Charisma damage.
Stage 2. 1 Intelligence damage, 1 Wisdom damage, 1 Charisma damage, and the Stupefied 1 condition.
Stage 3. 1d4 Intelligence damage, 1d4 Wisdom damage, 1d4 Charisma damage, and the Stupefied 2 condition.
Rot

If a creature’s Strength Score is reduced to 0 while infected with Rot, they gain the Paralyzed condition. A creature with the Paralyzed condition from Rot can only remove the condition by increasing their Strength to a Score of 1 or more.
Stage 1. 1d4 Strength damage and the Enfeebled 1 condition.
Stage 2. 1d4 Strength damage and the Enfeebled 2 condition.
Stage 3. 2d4 Strength damage and the Enfeebled 3 condition.
Shakes

If a creature’s Dexterity Score is reduced to 0 while infected with Shakes, they gain the Paralyzed condition. A creature with the Paralyzed condition from Shakes can only remove the condition by increasing their Dexterity to a Score of 1 or more.
Stage 1. 1d4 Dexterity damage and the Sluggish 1 condition.
Stage 2. 1d4 Dexterity damage and the Sluggish 2 condition.
Stage 3. 2d4 Dexterity damage and the Sluggish 3 condition.
Withering

If a creature’s Constitution Score is reduced to 0 while infected with Withering, they gain the Dead condition. Unless a is creature restored to life through a method that removes all diseases prior to gaining the Dead condition, they must make a Fortitude save when they return to life or continue suffering from Withering at Stage 1.
Stage 1. 1d4 Constitution damage and the Drained 1 condition.
Stage 2. 1d4 Constitution damage and the Drained 2 condition.
Stage 3. 2d4 Constitution damage and the Drained 3 condition.
Heighten (+3): For each Spell Slot of the Heightened Spell Levels (7, 9), increase the damage die size.
Level 7, 8, or 9 Spell Slot. Increase all 1 damage to 1d4 and all d4s damage to d6s damage.
Level 10 Spell Slot. Increase all 1 damage to 1d6 and all d4s damage to d8s damage.
