Insect Form ~ Required Score: 6

polymorph, transmutation

  • Duration: 1 minute per Proficiency Score (D)

You transform into a Medium animal battle form. You count as an animal in addition to your normal tags while in this form. Your gear is absorbed into your form; the constant abilities of your gear still function but you cannot activate them. When you prepare this extract, you chose one of the following and gain the attacks, speeds, and special abilities listed:

Ant Speed 30 feet, Climb Speed 30 feet; Mandibles with a number of d6s bludgeoning damage equal to half your Proficiency Score.

Beetle Speed 25 feet; Mandibles with a number of d10s bludgeoning damage equal to half your Proficiency Score.

Centipede Speed 25 feet, Climb Speed 25 feet; Darkvision; Mandibles with a number of d8s piercing damage equal to half your Proficiency Score plus a number of 2 Persistent Damage equal to the damage dice with the poison tag.

Mantis Speed 40 feet; Scent; Foreleg with a number of d8s bludgeoning damage equal to half your Proficiency Score.

Scorpion Speed 40 feet; Darkvision, tremorsense 60 feet; Stinger with a number of d8s piercing damage equal to half your Proficiency Score plus a number of 1 Persistent Damage equal to the damage dice with the poison tag; Pincer (agile) with a number of d6s bludgeoning damage equal to half your Proficiency Score.

Spider Speed 25 feet, climb 25 feet; Darkvision; Fangs with a number of d6s piercing damage equal to half your Proficiency Score plus a number of 1 Persistent Damage equal to the damage dice with the poison tag; Web (range 20 feet) with no damage but inflicts the Entangled condition on target for a round.

You can choose a more specific type of animal to turn into (such as a ladybug or scarab for beetle). You also gain the following statistical changes:

  • Add a +3 Dexterity bonus.
  • Add a +1 Natural Armor bonus to AC (size not included); ignore armor check penalty and reduced Speed.
  • Trained in natural attacks and you gain an extra +3 damage bonus.
  • Gain 5 temporary Hit Points while you have the form.
  • Low-light vision.
  • Gain advantage to Acrobatics.

The special statistics can be adjusted only by Insight bonuses, Assist bonuses, advantage, and disadvantage. Your form prevents you from crafting extracts, speaking, or using most actions with the manipulate tag that require hands (the GM determines which actions of this sort you can take if there is doubt).

Heighten (10, 14, 18): For each level, your form increases a size step and your attacks become a 10 foot reach. You must have enough space to expand into or the extract fails and is lost. Your Natural Armor bonus increases by 1 but for each size increase you take a size penalty to AC: –1 for Large, –2 for Huge, –4 for Gargantuan, –8 for Colossal; your damage bonus increases by 2; and your temporary Hit Points go up by 5.
